If your drivers are equipped with a Verizon Connect® Mobile™ app such as Verizon Connect® WorkPlan™, you can create template forms for messages that you want drivers to fill out. These forms are used in two ways:

When you dispatch jobs to a driver, the form can be associated with the job in the driver's list of jobs. In this case, the driver fills out the form when they perform the job.

If the driver is equipped with Verizon Connect® WorkPlan™, the forms are available for the driver to fill out on their own, optionally associating it with a job or an asset type.
From the Tasks section of the navigation bar choose Form Templates to open the Form Templates screen. Here you can define template forms that can be associated with job types.

Click Create to define a new form template. When you click this button, a new empty template, named "New" is added to the list of job form templates.

Click Delete to delete the currently selected job form template. When you delete a template, any association you have previously set up between that template and a job type is removed.

Click Import to import a standard form template to be used with your drivers. The Import a form template dialog box opens, listing all of the available built-in form templates. Select a template and click Import to import the template. Note that if you import a template that has the same name as one of the built-in form templates, that form template is replaced by the built-in form that you import.

Click Export to export a standard form template. The Export dialog box opens. Click OK to download the XML format file.

Click Clone to create a copy of the selected form template. The Name dialog box opens. Enter a name in the Template name field. By default, the clone is titled "Copy of [Source Template Name]".

Click Advanced to manage form template XSLT transform style files. The Manage custom stylesheet for [Source Template Name] dialog box opens.

Enter part or all of a form template name in the Filter field to filter the list of form templates. As you type the list is filtered, and lists only form template names that match your entered text.

Click Form Builder to display the controls for editing the currently selected form template.

Click Preview to see the form that the currently selected template creates, as it appears to your drivers.

Enter a name for the form template. This name is displayed at the top of the form. If you want to create a Pre DVIR or a Post DVIR form template, use the names "Pre DVIR" or "Post DVIR" (and the form type "Standard"). When these form names are used, the platform automatically recognizes these as DVIR forms.

Expand the Form Type drop-down menu and select the type of form to create: "Standard", "DVIR", or "DVIR Third Signature". DVIR (Driver Vehicle Inspection Report) forms are used for vehicle inspections, whereas standard forms are other types of mobile driver forms. "DVIR" is a basic vehicle inspection form. "DVIR Third Signature" is an inspection form that can be passed between a driver and a mechanic in three steps, when a vehicle fault is found. The options "DVIR" and "DVIR Third Signature" are option controlled. Without this option enabled in your account, neither controls are displayed in the drop-down list.

Click DVIR Walkaround Settings to open the DVIR Walkaround Settings dialog box. See Monitoring DVIR Walkaround Inspections for more information.

Expand the Carry over DVIR results drop-down menu and choose which DVIR results to carry over from one DVIR form to the next.

Click and drag on the bar that separates the two panels to resize them.

Use the controls in the form builder to add and configure the elements of your form template.

Click Save.
